Description

The project refers to the design and evaluation of both the implementation and impact of a multimodal intervention focused on the elderly patient living in the community (CARTS program), in order to prevent or delay their fragility, functional decline and risk of institutionalization, hospitalization and death. During the first phase, the development of interventions will take place, and then it will be the turn of the second phase, which will consist of a random clinical study with two parallel arms with one control group.

The participants in the control group will receive the ordinary assistance, consisting of the visits ad hoc by the primary health care teams, hospitalizations, common referrals or whatever is required by their health state. The people in the intervention group will be referred to a case manager nurse (or another health professional that is duly trained) for the initial application of a comprehensive geriatric assessment, and will then be prescribed the individual-focused interventions, depending on the different fields of action identified.

The project is developed within the Area of Primary Health Care of Barcelona city, with the multidisciplinary collaboration of the professionals themselves, the directorship and the research group FRAGICAP from the IDIAP Jordi Gol (Sr. Francesc Orfila), and its development is intended in various centers of the area of Barcelona in order to provide it with a higher external validity.
